1) Accomplished Unified Application form

2) Market Clearance (If market Stallholder)

2).Basis for computing taxes, fees, and charges (e.g. Gross Sales/Receipts/Financial Statements);

 3).Tax Clearance/Tax declaration or Contract of Lease (if Lessee); and copy of Business Permit of the lessor

4).Community Tax Certificate (cedula)

5)Barangay Business clearance

6).Sanitary Permit (Rural Health Unit)

7).Building Annual Inspection Certificate (Eng’g. Office)

8).Fire Safety Inspection Certificate or Certification of Non- Coverage or similar document of that Nature* (Bureau of Fire Protection)

Other applicable clearance, permits and certificates.

(5)   Occupancy Permit, if required by national law (e.g. Building Code and local laws),

(6)  Tax Clearance/Tax declaration or Contract of Lease (if Lessee); and copy of Business Permit of the lessor

(7)  Community Tax Certificate (cedula)

(8)  Barangay Business clearance

(9)  Locational Clearance (MPDOffice)

(10)  Sanitary Permit (Rural Health Unit)

(11)  Occupancy Permit  and Building Permit (Office of Building Official)

(12)  Fire Safety Inspection Certificate or Certification of Non- Coverage or similar document of that Nature* (Bureau of Fire Protection)
